Many people use cotton buds, earbuds, tissues, or cotton swabs to clean their ears without realizing that small pieces of cotton can accidentally become lodged deep inside the ear canal. While it may seem unlikely, a tiny piece of cotton can sometimes remain unnoticed in the ear for weeks, months, or even a year, especially if it does not initially cause significant symptoms.
In this article, Dr. Akshita Reddy, Consultant ENT Surgeon at ONUS Robotic Hospitals, explains how foreign bodies such as cotton can become trapped inside the ear, the symptoms they may cause, potential complications, and the safest treatment options.
What Is an Ear Foreign Body?
An ear foreign body refers to any object that becomes stuck inside the external ear canal. Common foreign bodies found in the ear include:
- Cotton from cotton buds or swabs
- Earplug fragments
- Insects
- Small beads and toys (especially in children)
- Pieces of tissue paper
- Hearing aid components
- Seeds or food particles
While some objects cause immediate discomfort, others may remain unnoticed for extended periods.
Can Cotton Stay in the Ear for a Year?
Yes. In certain cases, a small piece of cotton can remain trapped deep inside the ear canal for months or even years without the person realizing it.
The ear canal is relatively narrow, and cotton can become compacted near the eardrum. Because cotton is soft, it may not always produce severe pain immediately. However, over time, it can trap moisture, wax, and bacteria, increasing the risk of complications.
Many patients only discover the problem when they develop symptoms such as hearing loss, ear discharge, irritation, or recurrent ear infections.
Symptoms of Cotton Stuck in the Ear
The symptoms can vary depending on the size of the cotton piece, its location, and whether an infection has developed.
Common symptoms include:
1. Ear Blockage or Fullness
Patients often describe a sensation that the ear feels blocked or clogged.
2. Reduced Hearing
The cotton may obstruct sound transmission, resulting in temporary hearing loss.
3. Ear Pain
As the cotton presses against the ear canal or causes inflammation, discomfort or pain may develop.
4. Ear Discharge
Persistent discharge, sometimes foul-smelling, can occur if an infection develops around the foreign body.
5. Itching and Irritation
Continuous irritation of the ear canal may cause itching and discomfort.
6. Recurrent Ear Infections
Foreign bodies can trap moisture and bacteria, leading to repeated infections.
7. Ringing in the Ear
Some individuals may experience tinnitus or unusual sounds in the affected ear.
Why Is It Dangerous to Leave Cotton in the Ear?
A foreign body that remains in the ear for a prolonged period can lead to several complications:
- Chronic ear canal inflammation
- Bacterial or fungal infections
- Ear canal swelling
- Persistent discharge
- Wax accumulation around the cotton
- Hearing impairment
- Injury to the ear canal
- Rarely, damage to the eardrum
The longer the object remains inside the ear, the greater the likelihood of infection and discomfort.
Common Mistakes People Make
Many people attempt to remove cotton or other objects at home using:
- Cotton buds
- Hairpins
- Safety pins
- Matchsticks
- Tweezers
- Keys or sharp objects
These methods are strongly discouraged because they often push the cotton deeper into the ear canal and increase the risk of injury.
Attempting self-removal can result in:
- Bleeding
- Ear canal trauma
- Eardrum perforation
- Severe pain
- Worsening infection
