Sinus Headache: Causes, Symptoms, Diagnosis & Treatment- By Dr. Akshitha Reddy, ENT Specialist, ONUS Robotic Hospitals
A sinus headache is commonly felt as pain, heaviness, or pressure around the forehead, eyes, cheeks, nose, and face. Many people mistake it for a normal headache, but in some cases, it may be linked to sinus infection, blocked nose, allergies, nasal swelling, or chronic sinusitis.
Sinus-related facial pain usually occurs when the sinus spaces become inflamed or blocked. This can prevent proper drainage of mucus and create pressure around the face. Acute sinusitis commonly causes blocked or stuffy nose and may lead to pain or pressure around the eyes, cheeks, forehead, and nose.
What Is a Sinus Headache?
A sinus headache is a headache-like pain caused by inflammation or infection of the sinuses. The sinuses are air-filled spaces around the nose, forehead, cheeks, and eyes. When these spaces are swollen or blocked, patients may feel pressure, pain, heaviness, or fullness in the face.
Sinus headache pain is commonly felt in:
Forehead
Around the eyes
Cheeks
Bridge of the nose
Upper teeth or jaw area
Between the eyebrows
Cleveland Clinic describes sinus headache as facial pain that may feel like a constant dull ache behind the eyes, cheekbones, forehead, or bridge of the nose, and it may worsen when bending forward or moving the head suddenly.
Sinus Headache vs Normal Headache
A normal headache may occur due to stress, lack of sleep, dehydration, eye strain, migraine, tension, or other reasons. A sinus headache is more likely when headache is associated with nasal symptoms.
Sinus Headache May Include:
Blocked nose
Thick nasal discharge
Facial pressure
Pain around cheeks or forehead
Reduced smell
Post-nasal drip
Pain that worsens while bending forward
Cold or allergy symptoms
Fever in some cases
Normal Headache May Include:
Pain without blocked nose
Stress-related head tightness
Migraine-like throbbing pain
Sensitivity to light or sound
Nausea
Pain triggered by sleep loss or screen strain
Important point: Not every headache with facial pressure is sinus headache. Mayo Clinic notes that pain around the eyes, cheeks, and forehead may feel like sinusitis, but it can sometimes actually be migraine.
Common Symptoms of Sinusitis
Sinusitis symptoms may vary depending on whether the problem is acute or chronic. Common symptoms include:
Facial pain or pressure
Forehead heaviness
Pain around eyes or cheeks
Blocked or stuffy nose
Thick nasal discharge
Post-nasal drip
Reduced smell
Cough
Bad breath
Fever in acute infection
Fatigue
Ear pressure
Pain that worsens while bending forward
ENT Health notes that sinusitis may cause cloudy or colored nasal drainage along with blocked nose, facial pain, pressure, or fullness around the face, head, or eyes.
Common Causes of Sinus Headache
1. Sinus Infection
Sinus infection can occur after a cold, viral infection, or bacterial infection. The sinus openings may become blocked, causing mucus build-up and pressure.
2. Allergies
Dust allergy, pollen allergy, pollution, weather changes, and other allergens can cause nasal swelling and sinus blockage.
3. Blocked Nose
A blocked nose can prevent proper sinus drainage and lead to pressure symptoms.
4. Chronic Sinusitis
Chronic sinusitis is long-term inflammation of the sinuses that lasts for 12 weeks or longer despite treatment. It can cause nasal blockage, facial pressure, reduced smell, and repeated discomfort.
5. Deviated Nasal Septum or Nasal Polyps
Structural problems like deviated septum or nasal polyps may block sinus drainage and contribute to repeated sinus problems.
6. Pollution and Irritants
Smoke, dust, strong smells, chemical exposure, and pollution can irritate the nose and sinuses.
When Should You Consult an ENT Specialist?
Consult an ENT specialist if you have:
Headache with blocked nose
Facial pressure lasting several days
Repeated sinus headaches
Thick yellow or green nasal discharge
Fever with facial pain
Reduced smell
Symptoms not improving after 10 days
Symptoms improving first and then worsening again
Severe pain around eyes or forehead
Frequent allergy-related sinus issues
Chronic nasal blockage
Symptoms that do not improve after 10 days, or symptoms that improve and then worsen again, can suggest sinusitis that needs medical evaluation.
How Is Sinus Headache Diagnosed?
Diagnosis begins with a detailed ENT examination. The doctor may check the nose, throat, sinus tenderness, nasal blockage, allergies, and discharge.
Depending on symptoms, tests may include:
Nasal endoscopy
Clinical ENT examination
Allergy evaluation
CT scan of paranasal sinuses in selected cases
Evaluation for deviated septum or nasal polyps
Assessment to rule out migraine or other headache causes
Proper diagnosis is important because sinus headache, migraine, tension headache, and allergy-related headache can overlap.
Treatment Options for Sinus Headache
Treatment depends on the underlying cause.
1. Steam Inhalation and Hydration
Steam inhalation and good hydration may help loosen mucus and reduce nasal blockage in mild cases.
2. Saline Nasal Rinse
Saline nasal wash may help clear mucus, allergens, and irritants from the nose.
3. Medicines
Depending on the condition, the doctor may prescribe nasal sprays, antihistamines, decongestants, pain-relief medicines, or antibiotics only when bacterial infection is suspected.
4. Allergy Management
If allergies are causing repeated sinus symptoms, allergy control is important. Avoiding triggers, medicines, and nasal sprays may help.
5. Treatment for Chronic Sinusitis
Chronic sinusitis may need long-term medical management, nasal sprays, evaluation for polyps, and sometimes advanced ENT treatment.
6. Surgery When Required
Surgery is not needed for every sinus problem. It may be considered if there is chronic sinus blockage, nasal polyps, deviated septum, or repeated sinus infections not improving with medical treatment.
Warning Signs You Should Not Ignore
Seek urgent medical care if sinus headache is associated with:
Swelling around the eyes
Vision changes
Severe headache
High fever
Confusion
Neck stiffness
Persistent vomiting
