What Is Painful Arc Syndrome?

Painful Arc Syndrome is a shoulder condition where pain occurs only within a specific range of arm elevation — typically between 60° and 120°. The pain appears during active movement and reduces when lowering the arm. This classic pattern helps differentiate it from other types of shoulder pain.

Why Does the Pain Occur?

The shoulder has a narrow space called the subacromial space. When you lift your arm, the rotator cuff tendons and the subacromial bursa pass through this space.
If these structures are inflamed, swollen, or irritated, they get pinched — leading to sharp pain in the mid-range of arm movement.

Common Causes

1. Rotator Cuff Tendinitis

Inflammation of the supraspinatus tendon is the most common cause.

2. Shoulder Impingement Syndrome

Repetitive overhead work or sports reduces subacromial space.

3. Subacromial Bursitis

The bursa becomes irritated, increasing friction during movement.

4. AC Joint Arthritis

Bone spurs reduce movement space.

5. Rotator Cuff Tears
